Perhaps you have browsed our website or visited with a current member. If you think you might be interested in membership for the upcoming semester, please complete and submit the Prospective Member Form.

The deadline to submit your form for the Fall 2013 semester is Tuesday, July 9, 2013.

After receiving your form, we will contact you and schedule an interview to explain the ministry of CHFM in more detail.

Final interviews for the fall semester will be Wednesday, July 10, 2013.
